Safety Incidents
An accident of ‘Trip and Fall’ happened in the open stairs leading to Main Building, Lift Lobby ‘B’ and Warehouse Loading Bay Area on 16th January 2018 around 2:00pm, resulted in left knee injury to the employee. What Happened: An employee tripped and fell on the last step, while ascending the stairs mentioned above. How it Happened: While the employee was climbing up the stairs, she did not raise her left leg enough to reach the next step causing her to trip and fall at the edge of the last step. The fall resulted in redness and swelling on her left knee. Type of Accident: Lost Workday Case (LWC) with 1 day Medical Leave. Investigation shows that the main cause of the accident were “eyes not on path” and “not holding on the handrail”. Both are unsafe behaviors that many of us do not pay attention.
Lost Work Day was recorded. On 05th Oct 2017 around 1953 hrs, at Annex Bldg Staircase B. Employee slipped and fell on the last few steps of the stairs and injured her right knee. Prior to the incident , employee had been experiencing slight right ankle pain on the day. At the end of the shift, while she was descending the staircase, she felt a sharp pain on her right ankle and slipped few steps down the stairs. As a result, she injured her right knee. Initial Investigation shows that: - Floor was dry and no obstruction at the staircase - Employee was holding her bag on her left hand while supporting her sweater that was placed on her right shoulder while coming down the staircase - Employee had a big foot step when she stepped down the stairs
On 22nd July 2017 around 12:30 pm at Staircase A Level 3. Employee sustained abrasion and twisted his left ankle when he missed a step, while going down the staircase. Employee stepped out from MBB Production for his lunch and used the staircase to go to the canteen. While going down the staircase, he missed a step and landed with his left foot on a twisted angle. Be cautious and alert. Anticipate the hazards as you walk - Avoid rushing and pay attention to your surroundings - Grasp the handrail while both ascending or descending stairs - Do not look at your hand phone while walking - Avoid carrying objects with both hands - Do not carry bulky objects that block your vision
On 4th April at around 10am. An employee was rinsing his cup and spoon at Main Building BGA office pantry under the hot water dispenser after a cup of coffee. He was conversing with his two colleagues when he accidentally placed his right hand directly under the hot water which caused the scalding. Immediately, he retracted his right hand away from the running hot water. His right hand was swollen and seek medical treatment at the in-house clinic. Two days medical leave was given by the doctor. Safety Scoreboard had been reset zero.
Distractions are always the cause of many workplace injuries. Please be extra cautious and stay focused.
September 21 2015 afternoon, there was a minor slip and fall accident outside the main building near the liquid N2 tank area. It was raining and one of our employees was rushing to return to the main building when he slipped and fell along the ramp. Immediate medical treatment was provided at the Clinic for the wound he sustained on his right knee. EHS team would like to remind all to be extra cautious of the floor condition especially during rainy season as the floor is wet and slippery. Avoid running. Instead, walk briskly and pay attention to where you are going. In view of this accident, our safety scoreboard will be reset to zero today to reflect the accident.

August 2015, we had an accident at the Wafer Fab near the Plating station area. One of our employees tripped and fell on the floor. She was sent to the hospital via ambulance for further treatment. There was no serious injury suffered by the employee. In view of this accident, our safety scoreboard has been reset to zero on, 11

What can we do to avoid trip and fall?
➢
Be cautious and alert. Anticipate the hazards as you walk.
➢
Take your time and pay attention to where you are going.
➢
Adjust your steps to pace that is suitable for the walking surface and the tasks you are doing. When you come to a corner, slow down.
➢
Keep walking areas clear from clutter or obstructions.
➢
Do not look at your hand-phone while walking.
On 27
th
March, we had an accident at Wafer Fab along the Plating Fab link way. One of our employees tripped and fell on the floor. She fell and landed on her left elbow while her chin hit onto the black box she was carrying. In-house doctor was called for assistance and she was sent later to the hospital by an ambulance for further treatment. There was no serious injury suffered by the employee. To avoid trip and fall, everyone is reminded to be cautious and alert. Adjust your steps to the pace that is suitable for the walking surface and task that you are doing. Do not look at your hand-phone while walking to avoid such similar accident.

February, there was a minor accident involving one of our employees that resulted to mild scalding on her left front foot. It happened when she was filling her drinking bottle with hot water and when she was about to remove it, the mouth of the bottle hit the spout of the water dispenser which caused it to drop and hot water spilled onto her front foot. The employee received first aid treatment at our clinic.
May we remind everyone once again to be cautious when filling your bottle or cup with hot drinks. Release the hot water dispenser lever before you remove your bottle/cup and ensure that there is a clearance between your bottle/cup and the spout of the dispenser before you remove it.

November 2014, a Maintenance Personnel from Saber Engineering was pulling out a manual lifter loaded with used laser tube weighing about 200-300kg when his right toe was rolled over by the lifter wheel. He was sent to the in-house clinic for first aid treatment and was further advised to proceed to the hospital for additional treatment. We would like to advise everyone carrying out similar work activities to be vigilant and conduct thorough risk assessments in order to identify and implement appropriate safety measures to prevent accidents. In view of this accident, we have reset today our Safety Scoreboard to zero. Please help to disseminate this information to your team. After this incident’s full investigation, we will share more information.

Safety Scoreboard has been reset to zero last 9

July 2014 due to the accident that happened at Saber VI Changing Room when one of the employees slipped and fell on the floor. Everyone is reminded to be extra cautious and walk slowly when walking with their socks on while inside cleaning room changing room. As we aspire to improve our safety culture, we are encouraging employees to feedback and report to us all workplace related accidents including first aid cases and near misses. With this proactive approach, we can achieve and maintain a safe and healthy work environment for everyone.

(Nov-22) - Cold Shutdown of TFFC, from 25th Nov to 8th Dec: This will impact all of GGI, Saber and Saber VI. All tools will be powered down to maximize the cost avoidance. Facilities team to assist as required during the shutdown period.

It is the highest award level recognizing our facility for excellent green features, namely energy & water conservation, waste recycling and green consciousness. The award presentation ceremony will happen in Q2 2015. Thank-you to all for the hard work & commitment. We can be very proud of the award, especially it is our 3

National award in 2 years. The first two awards being Water Efficient Building Gold and Watermark award from PUB.

August we had reset our safety scoreboard to zero as a result of a minor accident at our Annex Building Office Pantry. One of our employees had a mild scalding on her right hand when she accidentally spilled her cup of coffee while walking out of the pantry. The employee received first aid treatment at our clinic. May we remind everyone to be extra careful when dispensing liquids or holding containers with hot liquids to avoid such similar incident.
Facilities has successfully concluded the One Lighting Certificate (OLC) audit that lasted for 3 days by SGS (auditor) with ZERO non-conformance and 8 OFIs (Opportunities for Improvement). Having the certification for ISO 14001- Environment and OHSAS 18001- Occupational Health and Safety under a Global certification. Audit scope is not only site specific but expanded to include EHS management system implementation which is linked to the Lighting Sector sustainability objectives. In other words, new audit scope is wider as compared to the previous years’ audit. Facilities has also conducted its 1

surveillance audit for SS 577 – Water Efficiency Management System together with the OLC audit. We are glad to inform, we passed with ZERO non-conformance and 4 OFIs. Outstanding achievement to the EHS team , EHS Steering committee, all EHS representatives, internal auditors and auditees who worked very hard in maintaining and improving the EHS and Water Effiiency management system.

May 2014. Throughout the game, Team encountered technical issues on the lane. Despite that Team played well and morale is high.
